Medical Power of Attorney

Medical Power of Attorney– This service is typically hired proactively so that clients will have a trusted health care agent to act on their behalf if and when they become unable to make their own medical decisions.

When we act as Medical Power of Attorney it is crucial that we understand all aspects of our client’s advanced care planning wishes. We achieve this through an initial comprehensive assessment. This gives us information about our client’s situation and includes a thorough discussion of our client’s plans and desires for the future.

After this step is complete, we hold biannual face-to-face meetings with our client. This allows us to regularly review our client’s advanced care planning, and ensures that we always have the most up-to-date information about our client’s wishes.

Should the need arise for us to act as Medical Power of Attorney we are able to:

  • Arrange services and appointments
  • Attend client appointments
  • Provide phone consultations with the client, family members, and professionals or staff involved in their care
  • Provide client well check visits at their residence, hospital, or other locations
  • Consult with professionals and staff care providers on the client’s behalf
  • Attend care conferencing and other meetings directly concerning the client’s care
  • Research information on the client’s behalf including: prescription plans, medical insurance, transportation services, housing options, etc.
  • Provide additional services as requested by the client such as: shopping, transportation to and from appointments, pickup and delivery of prescriptions, etc.
