Guardianship Services

Guardianship Services– These services are for those whom the court determines are no longer able to care for their own person and/or property. As a Legal Guardian, A Care Management Group has the duty to care for the personal, medical and legal interests of the Client. This includes making health care decisions, such as the hiring and coordination of all medical and non-medical care providers, and decisions regarding appropriate living arrangements. To meet our client’s needs we may:

  • Attend hearings and court appearances on behalf of the client
  • Provide court testimony in the client’s Guardianship case and prepare court mandated Guardianship Reports
  • Provide phone consultations with the client, their family members, and professionals or staff involved in their care
  • Provide regular in-person visits with the client at their residence, hospital, or other location
  • Consult professionals and care providers on the client’s behalf
  • Attend care conferencing and other meetings directly concerning the client’s medical care
  • Coordinate with other professionals, including, court appointed Conservators, agents under Power of Attorney, Trustees, or attorneys
  • Research information on the client’s behalf including: prescription plans, medical insurance, transportation services, housing options, care providers, or other topics the client’s needs require
  • Directly provide or coordinate other services to benefit the client such as: shopping, transportation to and from appointments, pickup and delivery of prescriptions, etc.

Please note: Although ACMG offers/provides both Guardianship Services & Conservatorship Services as a matter of policy we do not provide both services for the same individual.
