Adult Care Management

Adult Care Management– Our adult care management process starts with an assessment of our client. This allows us to understand our client’s needs and establish the next steps for their care management. After the assessment we offer on-going planning and coordination of care. We have a wide variety of services available to meet each client’s needs including but not limited to:

• Arranging services, and appointments
• Attending client appointments
• Provide phone consultations with the client, family members, and professionals or staff involved in their care
• Client well check visits at their residence, hospital, or other location
• Consultations with professionals and staff care providers on the client’s behalf
• Attending care conferencing and other meetings directly concerning the client’s care
• Researching information on the client’s behalf including: prescription plans, medical insurance, transportation services, housing options, etc.
• Additional services as requested by the client such as: shopping, transportation to and from appointments, pickup and delivery of prescriptions, etc.
