Initial Care Assessment & Comprehensive Care Assessment Report

Initial Care Assessment- Our Initial Care Assessment is a thorough evaluation of our client’s current condition and needs. A Care Assessment can be requested by a client themself, a client’s family, the court system or other professional.

To gather all the information needed to best serve our clients, our assessment process often includes several in person visits with our client. We may also speak with family members, healthcare professionals and any others with knowledge of the client’s current situation. We review a wide variety of information about our client, including health history and nutritional needs, social history, the client’s ability to manage the activities of daily living, home safety, and any demographic data. The exact information reviewed will depend on each client’s unique situation.

We provide our client with a final report which may include recommendations and referrals, possible next steps for our client, options for housing or facility placement, and information on various types of health and social care resources that may be of assistance to our client and their families. The exact information included in the report will be tailored to each client’s unique situation. This information allows our clients to make informed decisions as they pick the direction they want to move in or address changes they need to make.

Comprehensive Care Assessment Report– A Comprehensive Care Assessment Report may be requested in addition to the Initial Care Assessment. This report includes in-depth information about the health, nutrition, social welfare, and abilities of the older adult. This report is usually requested by the court system or a healthcare professional.
