What is Care Management?
Navigating the complex situations that occur as people age has become increasingly difficult for families to manage. Some of these situations are part of the normal aging process, some may be from disability or chronic illness. Our Care Managers work directly with clients and their families to deal with these real life problems.
We work hard to help our clients remain as autonomous as possible, but are able to assist in the areas where they cannot be independent. To meet this wide variety of needs we cover many different situations as part of our care management process. We have the ability to:
- Provide care planning assessments
- Schedule and attend doctors appointments
- Advocate on our client’s behalf with doctors or other medical professionals
- Screen and coordinate in home help or other support services
- Manage medications
- Provide transportation to and from appointments
- Act as a liaison with our client’s insurance company
- Help determine eligibility and apply for different types of assistance programs
- Review medical issues
- Provide health crisis intervention
- Act as a liaison for families at a distance, and alert family of any issues
- Provide well check visits at a clients home, hospital, or other locations
- Assist with living arrangements, including assisted living, memory care and skilled nursing facilities
- Help clients manage their living environment to maximize independence
- Assess client’s living arrangements for safety concerns
- Review financial or legal issues
- Pay bills on our client’s behalf or assist with financial paperwork
- Provide research on medical, financial, or legal issues
- Provide professional advice and recommendations
- Provide client and family education
- We can also provide additional services as requested by our clients such as: shopping, pickup and delivery of prescriptions, etc.
